If you see this Warning when trying to admit a patient into EQRS...
- Search for the patient to make sure they are not already admitted.
- Check the SSN.
- Check the MBI on the Medicare Card.
- Check if patient name is switched or misspelled (‘Bob’ or ‘ROBERT’ instead of ‘Robert’).
- Check if patient has 2 last names (with or without a “ – “ separating them).
- Check if date of birth is ‘off’ by one number.
- Use the correct Admit Reason: https://help.esrd.ipro.org/support/solutions/articles/9000233145-eqrs-admit-and-discharge-reasons-ipro-resource.
- CMS hasAdmission Scenarios that will also help.
If you are still unable to Admit the patient:
Submit the 'Possible Duplicate Form' and the Network will admit patient into your facility/center:
