EQRS Fixing System Discharges (and Transplant Non Functioning)

When your patient record shows a System Discharge, it means that another facility has admitted your patient - before your facility had a chance to enter the Discharge Reason.


1. Log into EQRS https://eqrs.cms.gov/globalapp/

2. Find the patient

3. Click Admissions

4. Click on the Admit Date for your facility record that shows the System Discharge



5. Click EDIT

6. Select the correct Discharge Reason: https://help.esrd.ipro.org/support/solutions/articles/9000015807-eqrs-admit-and-discharge-reasons

7. Update the Discharge Date if needed

8. Click Submit



9. If you are from a Transplant Center and the Discharge Reason is Transplant Failed...

10. Click on Treatments

11. Click on the Admit Date for your Transplant Center

12. Click Edit

13. Change the Transplant Status to 'Non-Functioning'

14. Click Submit
