1. Log into EQRS with your HARP Account.
If you do not have the QIP option: apply for access following these instructions: https://help.esrd.ipro.org/support/solutions/articles/9000183608-harp-eqrs-qip-access-dialysis-facilities
2. Click on 'Change Organization' to switch from EQRS to QIP
3. Select your facility
4. Select QIP Scores in the top Left
View/Download Reports to access your QIP Preview Score and Patient List of patients being counted in each measure.
