Closing a Facility / Interruption of Service

CMS requires that facilities notify the ESRD Network about any:

Change in shifts
Temporary closure
Permanent closure

Complete the IPRO Operations Status Form: https://redcap.ipro.org/surveys/?s=R8K7RWETHM
- Submit a copy of the Closure Letter on corporate letterhead (that includes the official Closure Date).
Make sure all patients are discharged from the facility before the Closure Date.

If dialysis staff or patients require assistance, please contact Network Patient Services: 516-231-9767.

The Network will work with you to ensure that all patients are transferred to other facilities and that your data is closed out of EQRS, the ESRD Quality Data Reporting System.
