EQRS Transplant Waitlist Dashboard

Facilities can view in EQRS the real-time patient transplant waitlist status as it appears in UNOS (United Network of Organ Sharing).

  1. Log into EQRS https://eqrs.cms.gov/globalapp/
  2. Scroll to the bottom of the Dashboard to the section: Dialysis Facility Transplant Waitlist.

  1. On the right you will see all your facility patients who have transplant waitlist information.
  2. Click Download CSV to get an Excel file of the full list.
  3. Type into Patient to search for a specific patient name.
  4. Hover over the Transplant Center column to get the contact information for the waitlisted transplant center.

How To Use This Information

  1. Identify which patients are missing from this Report and focus efforts towards getting them waitlisted:
    1. Contact the Network to request assistance from the Transplant Project Lead.
    2. Use and discuss Network resources with eligible patients.
    3. Identify barriers holding the patients back from getting on the waitlist.
  2. Review each listed patient’s Waitlist Status:
    1. Active: Patient is active on one (or multiple!) transplant waitlists – help them stay positive and support them in moving forward towards receiving a transplant.
    2. Removed: monitor the Removal Date and Removal Reason to determine if anything can be done to add patient back on the transplant waitlist.
    3. Temporarily Inactive: Talk with the Patient and Transplant Center to find ways to get the patient back on the Active list – so they do not miss out on an opportunity to receive a transplant!
