Please note:

  • Data is required to be submitted daily instead of monthly, the ESRD Network will process patient events reported through REDCap within 2 business days of receipt.  This will support in completing your 2728 and 2746 forms timely, meeting CMS guidelines.  
  • Verify patient roster on a monthly basis to ensure all patients are in EQRS.  
  • The link to the REDCap survey to submit patient event data is: IPRO ESRD Program Transplant Event Form.  Please save this link to your favorites.
